To find the information on your modem and the modem driver version.

Go in control panel / in System and select the tab Device Manager
Under Modem select modem and click on Propreties

In the new window select the tab Driver. There you will find the name of your
modem, the company (Provider) that made the micro-chip and the date of fabrication.
After, click on Driver File Details... to find the driver version.

The Driver version is in the bottom of the window.
